Cognición, depresión y estado funcional en adultos mayores de una comunidad rural de Actopan-Hidalgo, México

Evaluación del envejecimiento en comunidad-rural

Resumen

En México, existen pocos reportes sobre el estado cognitivo, funcional, anímico y de calidad de vida de los adultos mayores (AM) de las comunidades rurales. Este trabajo se propuso describir las relaciones entre escolaridad, edad, sexo, estado cognoscitivo, síntomas depresivos y grado de independencia para las actividades de la vida diaria asociadas con el envejecimiento, en un grupo de AM de una comunidad rural, con énfasis en las diferencias de género respecto a escolaridad. Participaron 94 AM (58 mujeres y 36 hombres) de 60 a 89 años, (edad promedio de 72.62 años). Se evaluaron las habilidades cognitivas (CASI), actividades básicas de la vida diaria (índice de Katz), actividades instrumentales de la vida diaria (índice Lawton y Brody) y presencia de depresión (GDS). El promedio de escolaridad de la muestra fue de 3.2 años. En el CASI, el 64.89% presentó puntajes alrededor de la media poblacional, el 16% obtuvo puntajes indicativos de posible deterioro cognitivo y el 19.15% obtuvo puntajes “normales” según los criterios publicados para este instrumento. El 16% de la muestra tuvo rendimiento cognitivo bajo, principalmente las mujeres con baja escolaridad. Se identificó una desigualdad de género reflejada en la baja escolaridad en las mujeres de 70-89 años, lo que posiblemente influye en su estado cognitivo. En contraste, los AM presentaron una baja incidencia de síntomas depresivos y puntajes que señalan independencia para realizar las actividades de la vida diaria, lo que posiblemente tiene relación con la actividad física que llevan a cabo en labores agrícolas y domésticas, y a una activa red de apoyo social detectada empíricamente dentro de esta comunidad. Para futuras generaciones, es deseable atenuar la desigualdad de género evidenciada en el presente estudio, mejorando el acceso a la educación y el nivel general de escolaridad, particularmente en las mujeres.
